yes BPS-21 is the highest grade in MLCG, and as far as your second question goes, I'm not sure about it. But in past MLCG had DMG officers and vice versa. I don't know the complete procedure or whether still it is like that or not. My dad was also offered a post of P.A in Bhutto regime, but he refused.
