What kind of medical fitness certificate is required for this post? And where can it be obtained from?

Further, it also states that it should be issued by a refigstered Medical Practitioner with apllication form. Does this mean any doctor or only a government doctor can issue this certificate.

What would be the contents of such a certificate IF there is no set format? What are the particulars that need to be in included in the certificate? Blood Group? Height? Weight?
