I also encountered the same kinda problem.
But resolved it manually myself and submitted the application successfully.

Here's the procedure that worked well for me:
First of all when you tick these options 'Yes', below appears respective forms.
You should enter date of the day when you filling the form wherever it is enquired.
In other options write None, No, etc, for example in Applied before, bank employee etc..
After adding all the information, then tick 'No' above the forms (which you previously selected 'Yes').
Hopefully you won't encounter any error then.
