bank challan form for the deposit of fee for css
 

i have completed the application bt cud nt get the challan form, where is it?i called in the NBP they r saying v do not provide any challan form, so u have 2 manage it urself, kindly help me in finding it. i have checked the advertisement n the website of fpsc a zillion tym bt there is no such info.
secondly plz provide me the head for challan form.

go to your local fpsc office and they will give it you for free, fill in your name , exam name , branch name and branch code and report at the nbp, submit 1500 PKR and the officer will stamp it coupled with writing chalan number on it. He will keep two copies with himself and return 2 copies to you. It will be FPSC copy and candidate copy. Now If you want to apply manually go back to FPSC office and show them these copies and they will hand over forms and syllabus and will stamp the copy with [U]form supplied so and so #.[/U] now fill the forms and attach the fpsc copy with it and submit it in fpsc office and keep the candidate copy with you. In case you want to apply online , when you the get the forms printed just attach the fpsc copy with it and courier it to fpsc head office.
