Default Why candidates fail in first attempt??

I want to know that why most of time candidates fail in first attempt. Then in second attempt they get success.. Why is that so??
What beginner aspirant could to avoid those fatal mistakes and get their goal in first attempt?

actually in first attempt they don't know how to attempt the paper,how to manage the time and the most important is how to write the quality content to get the marks.

It’s all about proper planning, practice and execution. One can qualify in first attempt through apt planning, rigorous practice and perfect execution. One will fail the exam even in third attempt if one of the aforementioned components is missing

By the way, i have one thing more to ask that before making notes one should read whole book or just recurring topics of subject? And yea any suggestion for CA?
