Default Required Documents and Application form Queries

Hi, few questions here regarding the application form.

1. My name on my Matric certificate is misspelt (long story) but I do have an affidavit stating the actual spelling. I used the affidavit while applying for study abroad and there wasn't any problem with getting admission into uni. My CNIC, BA Degree, Passport and birth report have the actual correct spelling. As per FPSC's guide for online application, the name entry should be in accordance with the Matric certificate. Should I write down my correct spelling and send in a photocopy of the affidavit? Also, in case, Equivalence is enough, would I need to send in the original document or attested copy?
2. Would I need to get a Minority certificate if I belong to a minority Muslim sect?
3. Does getting an Equivalence certificate mean having my foreign degree attested? Or is it something entirely separate since attested copies of degrees is a requirement?
4. This is probably a dumb question. Bare with me. When they say attested copy, do they mean photocopy of the attested document or photocopy of a document that needs to be attested? (I am asking because I had to attest photocopies before and Abu got copies of THOSE attested photocopies as well so just want to make sure if there is a way out of that khuwari).

I would be grateful if someone could help me out.
