Focus on your post graduation. You’re starting too late for CSS. It is wise now to stick to what you’re doing. Better still, try to move out of Pakistan (preferably to Europe, as you’ve already surpassed the appropriate age for USMLE).

Aoa
I think you can take exams this year.
4.5 months are there.

You are MA doctor ,and I suppose you are well at studies management.

CSS in nothing but studies management and self belief.

I would recommend you to first take firm decision, whether to take exams or not, then plan your studies.
What to do

Give 15 days to read all the tips given here on cssforum

Select courses that are trending

Plan how to prepare all the syllabus

Hope this helps.

Moreover, I would second party pooper its very risky to only rely on CSS, but as you are doctor I would recommend you to prepare these 4.5 months with full devotion and then after papers stay in your field.

Regards
