one sport kit or sport suit form


one bank particulars form

stamp paper with duplicate with photocopy of id card of mine my surety and witness

one certificate issued by a guzetted officer who certified that i am not bound by any similiar kind of bond

Three photocopies of my id card

and do i have to sign tht gazaated officer thing on a stam paper too?

duplicate means photocopy i mention in bracketts, i.e, u will also hv to send the photocopy of stamp paper
and cetificate will not be on stamp paper it should be in his letter pad supported with his stamp or on a plain paper supported by his stamp,its better to hv it on his letter pad.

AOA...

mny of our CATians have received letter from CSA but i didnt...i m little bit upset abt this as i have changed my adress but i had already informed tht change to FPSC and MINISTRY but they both first sent me letters on my old address... i worry tht CSA would do the same...

@ehsan
have u received letter from CSA...how can i confim from CSA abt my letter means any contact number of concerned person...

i also didnt received any bio data form in my form.. call deputy director, this is his no, he deals in our matters..042-99220455

ok i think that the CSA wd do the same it was better to rectify ur address in CSA documents as well but as they r bound to send the letters on thoz address provided to them so they will do accordingly,perhaps estb div or ur concerned ministry forwarded ur new address to CSA but its look quite uncertain,so do contact Mr irfan Dy Dir Coordination on 042-99220455 regarding updates,

yes i recved mine 4 days back,but don not be worry bcoz my 1 frnd from southern punjab recved it today,its means its under process but contact mr irfan abt ur address as soon as it possible

do we need to provide the copies of cnic of witnesses as well or their cnic no wud b enough?
really puzzled as one of my witness has gone to fasalabd 4 sm days n he wont b bak by 1 jan

yes the photostat of witness,surety and probationer is necessary,try ur best to find the photocopy of id card of ur witness,if incase u r wd not find it then change ur witness and hv new stamp paper and still if its not possible then send it without photocopy and gv it later to CSA but donot forget to send the photocopy of id card of ur surety,and urs and ur other witness
