Another correction I want to make is the sentence ...

"This is the historical spot where he was shot dead"

it should be

"This is the historic spot where he was shot"

For the proper use of Historic/Historical please read the following passage

Historic and historical have different usages though their senses overlap. Historic refers to what is important in history: the historic first voyage to the Moon. It is also used of what is famous or interesting because of its association with persons or events in history: a historic house. Historical refers to whatever existed in the past, whether regarded as important or not: a minor historical character. Historical also refers to anything concerned with history or the study of the past: a historical novel, historical discoveries. While these distinctions are useful, don’t be surprised if you see these words used interchangeably, as in historic times or historical times.

I haven't paid much attention to this part because my perception is that its how things should be used rightly or you just know when something is used wrong but is there a procedure or hints or steps to follow to figure out errors?

Does anyone have solved examples of direct/indirect and active/passive voice that they can share?

there is the mistake of verb.. but i am confused too. so if any one knows than please correct it with reasons.

you seem to be right. beacuse when we use "as well as"

the verb should agree to the noun before "as well as"

now please correct me if i am wrong.
