Outline matters a lot in essay paper.
Examiner don't have enough time to go through every paper thoroughly, i am saying this based on the papers he checked this year, he goes through the papers of those candidates who have potential in their outline.
your outline consist 9 segments that are related to pollution only, instead the essay requires ways to combat it. This is the main mistake you have committed.

Sir, it's 40 marks essay NOT 100. Secondly, some seniors recommend short outline rather longer one but the logic you've suggested is really appealing. Although I wrote a longer outline on rough but while copying, I just shortened it for the sake of experiment as some of the qualifiers suggest so. However, I qualified PMS as well as CE-2015 Essays with long outlines rather shorter one which rationally follows your suggestion.

If you can provide the crux of your point in three or four words then your outline will be more qualitative. Which is very good. But if you cannot do it or if you cannot find good words then a long and clear sentence is also acceptable in outline. As you have cleared, you must know it better.
Marking it out of 40 then you won't get 16 because majority of your topic is on 'pollution' rather than on the 'ways to combat it.'
