Your outline is ok and passing. Usually, examiners pass candidates with cause, effect and remedy pattern for this type of problematic essays.

Dear San, Hard luck with your US history, but I am sure you will do great next time. Regarding your essay I liked the fact that the English is simple and the outline very clear, and in my opinion, it was a good attempt you made there.

I have a few questions about the essay in case you can help me:

1. Did you use these outlines in your final essay too?
2. Did you make the lists/bullets in the essay?
3. How many words you wrote in total (estimated)
4. How did you put quotes in the essay? (indented/italics/centered)

It will be great help if I can get guidance from someone first hand who cleared the essay.

No, you can not make bullets in essay except in outline.
I wrote around 1500 words and got 41 marks.
As far as quotes are concerned, you can put in accordance with logic. In some paragraphs, I wrote them in begining and in others, at the end.
