How to use quotes?

Quoted are very important in PA and highly recommended. But it is confusing on how to use them in our answer.

Should we use the source of quotes as well? e.g.

Quaid said in his address on dd.mm.yyyy that.................!

Now, should we also use the source of the document on which this statement is found?

I was told by some teacher that its not that much important the date but its important the source i.e., the Author Name.

Example 3 :
"It's demeaning when people talk about my looks. I think I usually look like shit, and most people would agree." (Johnny Depp)


You may also indicate the Date, but it must be correct and i would prefer to write Dates especially in PA Paper. Atleast you should write the name of the Author.
