If you want to apply through proper channel i-e if you want that your current experience is counted in your total government service after selection through CSS then you have to submit departmental permission certificate.
you cannot claim age relaxation for less than two years govt service.

Branch: Name of the Branch of National Bank where you deposit the fee.

Branch code: code of that branch (You can ask it from bank while depositing the fee)

Same was the case with me in CE-2013. I just wrote the result year and left rest of other columns blank. They have not asked me about the result date till today
