Query regarding attestation of O and A level equivalence certificate

I want to know what documents an O/A level qualified student is required to attach with his CSS application form. I've heard that we are required to attach 'attested' photocopies of our equivalence certificate. Do we have to get copies of our equivalence certificates attested by any grade 17 or above officer or by IBCC.
I've just graduated and I've not received my degree certificate yet all I have with me is a provisional certificate/transcript. would it be acceptable? if yes then from whom should I get its copy attested.
It'd be really kind if anyone guide me regarding complete application process that an O/A level student has to follow.

If you got any answers to your queries from a source other than this forum, please let me know. I had no idea that we need to get equivalence certificates for O/A level, just hearing of this now and sort of freaking out about it, since there's only a few days left for the application deadline now.
