Apne record k lye TR Slip ki photo copy kra k apne pass rakhe aur original bank stamp wali slip FPSC ko bhje...

I sent my application on 16th Oct but haven't received the receipt of receiving from FPSC.

Enter only one thing..either cgpa or grade

You won't receive it... No one will.

You can confirm its reception at FPSC Islamabad through TCS Tracking... provided that you've sent it through TCS and no other service. TCS has the option of online tracking your shipment through consignment note number at the top of your TCS slip...

My domicile district is Gujranwala but I mistakenly entered Gujrat instead of Gujranwala and have applied for css. Is there any way to correct it?

Make that correction on hard copy with pen and attach an application mentioning the mishap...
