Do we have to take 4 photo copies of the clearance proforma by filling one or do we have to separately fill four copies and submit them?

I made both the arrangements. Photocopies from one copy and also filled four separate forms. However, on the day of psychological photocopies were not taken from me, though they took the 4 separately filled ones from me. (Peshawar Center)

So it means photocopies are not required. Fill four security forms.

i have received 4 final preference forms and they had asked for 5..what should i do?

Well for any application form you have to allow the photo of your at the size of passport size..
