Achievments of the Holy Prophet(P.B.U.H) as a reformer:
1.united the scattered tribes
2.prohibited the sedition system
3.introduced equality among human biengs
4.end the tyranny from society
5.provide fundamental rights of islam
6.set judicial system
7.ignorance was come to an end
8.Co_operation
9.social respect
10.golden rules for daily life
11.protect the life,priperty and respect of others
12.good behaviour towards fellow beings

well, Although I am not a senior, humble suggestion would be: how about dividing them under following headings?
Religious reforms
Political reforms
Social reforms
Economic reforms

than further one may produce headings according to main heading.
as in religious reforms
shift from polytheism to monotheism.
one may further illustrate the 4 pillars of islam
freedom for all religions

under economical
zakat structure
prohibition of usury
different terms of islamic trading i.e: murabaha, mudarabah, ijarah etc

and so on

introduction of Majlis-e-Shura
fine aspects of zakat
right of texation on riches etc
is it good?

I think zakat should be under economical reforms and tax or jizya also should be there. under political you may include the first constitution of madina and its wide acceptance by all religions, pact of hudaibia should be also considered as a political reform, and Invitation toward Islam to the great powers of that time which was a new step in international Relations, military reforms could be also another heading.

RELIGIOUS
1.Unity of ALLAh and obedience of the prophet.
2. Religious freedom for all
3.reverence to all religions
4.Ended jahilya ,worship period

POLITICAL
1. Sovereignty of ALLAH and prophet's leadership
2. Ended blood feuds
3. Introduced politics of reconciliation than politics of vengeance
4. Democratic principles: accountability, shura, consultation, good governance.Formed an impartial systematic law in Medina guaranteeing equal rights to all

Social
1.emancipation of slaves
2. exalted women's status
3. Education stressed
4. Social vices abolished
5. Virtues and morals replaced
6. Equality and brotherhood emphasised
7. Hilf al fudul
8. Ethical and moral teachings that brought social revolution: Respect for parents, Love for Children, Forgiveness, Compassion and mercy, Care for neighbours, human rights.

Economic
1. Equitable distribution of resources
2. Revenue system
3. Created distinction between halal and haram
4. Ethics and morals combined with business and trade
5. Usury abolished
6. Individual allowed to own property without state's interference
