Clinical diagnoses is mixture of all these or you can say that these are strategies to find out diagnosis. So u always find these separately instead of any case history, might be that give you full information at one place. We take Behavioral assessment, clinical interview n mental status examination in intake and intelligence and neuro psychological assessment are the part of testing phase, By the result of all these we get clinical diagnosis.

There are several definitions of abnormality, each having its own flaws. None is perfect or universally accepted.
1:
deviation from normality is abnormality.
limitation: if corruption is normality in a society, is being fair abnormality??
2:
lake of ability to adjust is abnormality.
Limitation: one can adjust easily in one's own society but.... so....
3:
Psychological health and mental illness...
Limitation: how to know who is psychologically unhealthy??

Interview
Observation
Different tests etc...
Clinical judgement leads to right diagnosis and only right diagnosis can lead to right treatment.

DSM IV is a reliable source of information for a clinical diagnosis.
But for some culturally biased disorders one have to look for one's culture and norms...
