Though majority of people attempted first essay, but it was the only essay that provided you with material enough for writing 2500-3000 words without the risk of going irrelevant. But who ever tried to limit its scope, whether for Freedom of Speech or just IDPs, didn't answer the question.

Introduction
9/11 and us invasion in afganistan
Present position
Us failure
Is war on terror limited to Afghanistan?
War on terror in iraq Syria Middle East
ISIS

this has nothing to do with topic. why this?
Human Rights violation

American violation of Humanitarian Laws
Attrocities in Afghanistan
Poverty
Inequalities
Food shortage
Refuge Camps
Internal Displacement
Killings of Innocents
peshawar Incident
Killings of Children
Intolerance
Sectarian Killings
Shikarpur Incident

can they be termed human rights violations??

No freedom of Expression
Media under threat
Child Abuse
Women victimizations
Minorities Under threat
Hurdles in admistration of Justice
Bad Governace
Hurdles in implementation of law

again, can they be termed human rights violations caused by war on terror??

Remedies:

21 Amendment
Military Courts (really? aint they creating another problem by not giving a fair trial?)
Zarb E Azab
Banned On Capital punishments uplifted

Conclusion

In my view most of this was irrelevent. it seems like you had tried to reach the word limit and have not understood what the topic demanded.

you dont need to explain to them what war is. nor do you need to explain to them what terror is. or, for thta matter, what war on terror is. what they have asked you is whether the war on terror ( as understood in the current scenario) has led to any human rights violations? and if you agree with that, what are those violations. in my opinion your outline is irrelevant to the topic.

Asslam-o-Alaikum There was very clearly written in the question that:
"1. Make sure you use different forms of discourses, e.g., exposition, argumentation, description and narration."
"2. Credit will be given for organization, relevance and clarity."
I think after reading and understanding above statements there is nothing left for ambiguous questions that which outline is better and what to write and how to write. The only thing matters is ow you developed your thesis statement and expand essay accordingly.

I will try to write down outlines of all the topics soon and I will also explain method of writing and attempting an essay on the basis of my experience and my study.

and also keep one thing in mind, since about 90% people went for 'War on Terror' essay so only that will pass who would have chiseled his essay exactly according to the exact scope of the topic as only one teacher would check all the essay papers !!!..
