Simply fill out the form and send it via post. They didn't mention anything else on ad or their website, so hopefully that's all they want. There will be no test just interviews of shortlisted candidates so no test fee or date etc. Just send your application and hope for the best! :)

For JE (Sr. No 2) and PA (Sr. No 3) YES
For Scientific officer (Sr. No 1) NO
