Bro in health certificate there is a column Chest X-ray exam(attached chest X-ray report) and Laboratory exam (attached test report of AIDS, Syphilis etc). Do we have to attach X-ray report and Test report of AIDS with this health certificate? Plus is blood report also required with it?

fahad bhai, i took the medical certificate signed by the respective medical officer, but hospital did not attach any xray report and also they did not take my LDVR test, so i went to Lahore chugtai lab where i gave my blood sample for ldvr test, is it right??? and also you mentioned in your previous post notarized what does it meant for??and also last thing is it necessary to have recommendations letter to those who have taught you courses in your Masters or University it self by the Program manager can give it to you. kindly reply as it is the biggest tension for me in this limited time. looking for your response. thanks

Attaching Chughtai lab blood report wasn't necessary but if you have it and it is not inconsistent with the filled form then you can attach it(beside results, time and date should also be consistent). It was already mentioned in my previous post that attach whatever is returned to you(in form of paper document besides the filled foreigner physical examination form) by the hospital and you don't need to do anything extra. Notarizing is a type of attestation. Go to your nearest court and ask any stamp paper seller, he will get your documents notarized for you. Generally they take between Rs. 10 to Rs. 20 per document. For academic purposes, recommendation letters from academic staff are required i.e from faculty members of your last institute attended. CSC has specifically mentioned on its website that the recommendation letters should be signed by full professor or associate professor. But if you cannot acquire then give other sources a try, they may or may not be acceptable. It depends on policy of your prospective university. |

And do not follow the IPC instructions regarding the documents as they are unaware of the requirements of donor agencies. You should follow the CSC(Chinese Scholarship Council) instructions which is the donor agency, if you don't follow then it will simply reject the application. CSC requirements as mentioned on its website: Application Documents (in duplicate): 1. Application Form for Chinese Government Scholarship (written in Chinese or English); 2. Notarized highest diploma; Prospective diploma recipients must submit official document issued by your current school to prove your current student status or expected graduation date. Academic transcripts (written in Chinese or English); Transcripts in languages other than Chinese or English must be attached with notarized Chinese or English translations. 4. Two Recommendation Letters (written in Chinese or English); Applicants must submit two recommendation letters signed by a professor or an associate professor. 6. A CD of your own works (only required for arts students); Applicants for music studies are requested to submit a CD of their own musical works. Applicants for the fine arts programs must submit a CD of their own works which include two sketches, two color paintings and two other works. 7. Foreigner Physical Examination Form (photocopy) (written in English, can be downloaded from http://www.csc.edu.cn/laihua or http://www.campuschina.org); The physical examinations must cover all of the items listed in the Foreigner Physical Examination Form. Incomplete forms or forms without the signature of the attending physician, or the official stamp of the hospital, or a sealed photograph of the applicant are considered as invalid. Please carefully plan your physical examination schedule as the result is valid for only 6 months. 8. The copy of valid HSK Certificate (if available). All documents should be bound together on top left corner in the order of 6.1 to 6.8. (You should submit TWO sets of bound documents). No application documents will be returned. |

IPC does not publish merit list online. You can visit its office in Islamabad to see the merit list or you can call them and find out your merit position. They are old school like most of our governance system.
