No need to worry for that. They will accept the form as you have already mentioned your date of birth in the form.

HELP NEEDED
Currentliy i m living in isb and one of my friend has deposited fee in national bank branch but unfortunately there is no challan number mention......i was thinking to apply online..........so what to do as with out challan number i am unable to apply online......

i elect the subject in PMS KPK sociology,journalism,public administration, mercantile law,indo pk history,,but the option is appear in the online apply,, tat we can choose minimum 2 subject from each group,,,as i select these subject from different group but not more than 200 marks from each group. plz guide me some extent

KPK PSC rules regarding the optional says:
"The candidates are required to select optional subjects carrying a total of 600 marks, but not more than 200 marks from a single group"
Your selection of sociology,journalism,public administration, mercantile law,indo pak history is ok... No need to worry.
In online apply no problem occurred in selecting optionals. Your statement "that we can choose minimum 2 subject from each group" is incorrect. From each group we have to choose maximum 200 marks subject/subjects (keeping in mind the the restrictions, i.e, IR and IL can't be opt combine etc).
Kindly avoid net speak in future.
