How to get Physical Standard Documentary Proof

Dear All,
Its is not as easy as new candidates assume to get a documentary proof of Physical Standards from DHQ, Here is the correct order to get a physical standard certificate.

This certificate is issued after your physical assessment at DPO office.
Dont worry, here is the procedure.

Documents you would need:
a: 1 photograph
b: Original Documents and copies of Matric, Bachelors, Domicile, Advertisement of post and CNIC (original + 2 copies)
c: A pen to write an application

Step 1:
Go to your DHQ hospital and ask the clerk to issue a covering letter and a document asking the OASI in DPO Office to check your physical standard, take the advertisement with you showing the minimum required standards.

Step 2:
Now take that letter and form to DPO office in OASI branch, SP or DPO will call a committee of atleast 3 personals to check your height and Chest. They will write it down on that form marking with PASS or FAIL.

Step 3:
Now again back to DHQ for your vision test ( not mandatory but necessary for overall clearance) in Ophthalmology Department. Doctor will check your vision and you have to get the same certificate signed by Head of Department.

Step 4: Now take the signed form to MS of DHQ or THQ whatsoever and get it signed , Your Document is ready to dispatch now.

[B]You have to send Original Physical Standard Document with your application[/B]

[B]Any other piece of paper OR any signed and stamped document showing physical standards is not acceptable[/B]

have you been shortlisted first time? :)) .. you have made it [B][B]problem of kashmir[/B][/B].. Its not like that.. simply go to any govt hospital. Ask them to make a nominal medical mentioning height, weight and chest. This is it.

Well brother I am from Punjab and i am stating on the basis of my personal experience. It is the only way because many candidates were also there for fitness certificate for PPSC Sub Inspector post. Same procedure was adopted.

Well, i have done the same, went to Jinnah, and the Doctor over there of grade - 17 took the required measurements with the help of staff and issued the certificate on the hospital's letter with his name stamp and sign. That's it.

i have send photocopy of physical standard certificate not original ... wil it be acceptable or not?

I don't think so, but send original right away along with an application mentioning the reasons…

I think there is no need to upload the snap. letter pad of the Dr. in government hospital or even OPD (with entry number) slip will work.

[B][U]Format:[/U][/B]

It is certified that Mr.________ S/0 Mr._________ bearing CNIC__________, holds the measurement of following physical standards as

Height:
Chest:

then get it signed and stamped by the doctor, and counter signed and stamped by the Medical superintendent. If you have the reference then it is activity of 10 minutes only otherwise its herculean task.
