As per my knowledge, this will be a certificate issued by physician working in a government hospital. He will check your height, eyesight etc (whatever is required) and issue a certificate with his stamp on it.

But you don't need it now. It will be required after MCQs and descriptive test, and these two test will take about 6-8 months. So keep your focus on test preparation.
