You will receive test pattern with examination slip. Last year I appeared in this test and qualified the exam but didn't join due to some reasons. Test pattern was as follow:

100 marks MCQs Test: 100 questions.

1. Current Affairs: 20
2. Pakistan Affairs: 20
3. Islamiat: 20
4. English: 20
5. Arithmetics: 20

There were also some questions about Everyday Science.

You can get it from any Govt. Hospital or any doctor entitled to issue such certificates. Your height, Chest measurement and weight should be mentioned properly.
Yes brother it is clearly mentioned in NOTE:

Candidates are advised to submit their physical fitness certificate of prescribed standard issued by a registered Medical Practitioner with application form.

With very initial doc. Other requirements might be written on form.
i think the certificate is not mandatory, if so, we would be required not advised to submit it...

It would be better if you attach it with your application. Please note that on your reporting to the ASF Academy at Karachi, if selected, you will again be medically examined.

Its like Initial and Final medical that is the procedure of Armed Forces. If you don't submit the medical certificate, they can ask you for it personally and possibly your application can be rejected too. So, I think you should submit it with your application for getting rid of any future inconvenience.

I submitted this certificate with my application last time, and getting it was not a big deal.
