"From" mean when you got enrolled in your Matriculation and "To" means when it ended with your exam.
Bhale combine e kiya ho apne, just write date when you got enrolled in your 9th class and in "To" enter date of the matric exam.

No. You have to fill in only reuired details that include Bank details(Name,District,Code), candidate details and amount particulars(total amount and its description--CE-18 Registration Fees).
