(ii) Recent Passport size photographs (4 copies) mentioning name and father’s name on its back, attested by a Gazetted officer.
In this case i suggest you to paste one photgraph on form n attach 4 pics attested on its back alongwith other docements.

No in this case there is no need to fax the first page to FPSC Islamabad. Fax is only in those cases when you hve no facility to submit the form n fax fron page to FPSC Islamabad n then send the documents later as specified in their rules.
