Hi,
the best way would be to write to FPSC, on a plain paper along with your form, that your name has been spelled wrong mistakenly on some of your documents. that you have applied to get them corrected. and attach fotocopies of the receipts you have got when you applied for name correction at the relevant board/university. and that you will correct your new documents as soon as they become available. I believe they won't reject ur form
the only way they do so is when their is some illegality in your application. as your form has no problem other than the name correction, it won't affect your chances. People can even change their names after joining the CSA
