Furthermore for your above mentioned queries, my suggestions are:

1) Only practice (keeping some suggestions in mind) can help you in improving handwriting.

2) Better way of holding ball point pen is from the middle i.e. holding at drum area of pen, in such a way that drum is placed on middle finger and the index finger and thumb will be holding pen, neither too tight nor too loose. Initially you will face difficulty but with practice after some time you will be comfortable with that.

3) Use medium pointed ball pen. I found picasso ball pen best.
