Normally you do get the Chalan from inside/outside (people sitting outside with different forms) the National bank, and you can also download it from FPSC. Important thing is that you mention your name, father name, post applied for and the 'head' to which the fee is being submitted i.e (C02101.....). When you submit the fee you get a Chalan number and you can ask for branch code from the bank, fill up these details on-line and you are good to go
