Today I went NIRM G-8/2 Islamabad and they given a form of a private lab( located in blue area) along with their own form where it was mentioned all the tests and documents which i' ll provide them tomorrow.
I went that private Lab they charged me Rs. 1800. They got my blood , urine sample and x-ray of chest for the tests and said that reports will be there at NIRM Islamabad Tomorrow. Now I 'll have to appear before the Central Medical Board on 19-02-2013 along with following documents

1. Rs. 100/- treasury challan under hear of Health fee for medical Examination
2. A latest prescription from eye specialist with suitable glasses worn at the time of examination.
3. Two attested passport sized photographs.
4. Attested Copy of CNIC
5. Medical Board's Fee RS. 800/-
