i am gov employee, and my organization allow only one DEPARTMENT PERMISSION CERTIFICATE in a year, in 2014 i have already got one for A.D land records.

can i fill colum number 23 Employment Record:

and what about colum number 26. For Candidates Applying Through Proper Channel
