Saving news lines/articles/editorials etc in digitally is as good an option as maintaining a hardcopy.
45 minutes is not 1/3rd of the time..its more like 1/4th..If you have a comprehensive and properly made outline,you just have to fill it in the next 135 minutes.And this is more than enough for writing a good,to the point yet substantially long essay.Don't bother about writing 35 pages(its not a divine commandment).Write 20-22 pages of quality essay.This should do the job.If I wrote 27 pages in 135 minutes anyone can.I'm tellinh you this from my own humble experience.
You don't really need an outline for other papers.I used to spend 2 minutes at the most at the start of an answer.make a small rough outline on the back page.This was just to give some basic structure to my answer and to carve out a logical flow of the reply.If you remember a point you need to address while you are writing the answer,write it down in small form outside the margins.Helps you remember relevant arguments/points among a jumble of many.Giving headings is a smart ploy.But you should write quality and relevant material under a heading.Don't go on a 'Heading spree' like we used to do in the FSc.Some papers like History,Pk Affairs,Current Affairs demand more headings than other subjects like sociology,Islamiat etc.

I don't have my notes with me anymore.All distributed.Some of my students(if you can find any at this forum) at KIPS might have them.For most subjects,I made notes in point form which only I can comprehend.That's why I don't even know where they are now :p
