The Permanent Residence Certificate (PRC), as per my experience, is issued by the Union council (UC), the one where you are residing. Now this PRC thing basically has info on it but no picture. However, the form C and D are issued by the DCO office, showing that you are resident of the said district.
However, I don't see why in the first place you want a PRC, when you already have the domicile.
