Don't loose hope and for now just take the chance. Also apply directly to CSC as backup plan, in this case you will need to forward your CSC application to universities and fill their application forms as well as pay their application fee. You will get the scholarship one way or the other.

Just start to collect your academic documents like transcripts, recommendation letters, etc. Also write your statement of purpose. Go for medical after receiving. If you don't get the letter in few days then go to IPC after taking time and collect a new one from there.

There is no blood test form. Its the blood test report MS will recommend, based on which he will fill the foreigner physical examination form.
