This certificate is issued after your physical assessment at DPO office.
Dont worry, here is the procedure.
Documents you would need:
a: 1 photograph
b: Original Documents and copies of Matric, Bachelors, Domicile, Advertisement of post and CNIC (original + 2 copies)
c: A pen to write an application
Step 1:
Go to your DHQ hospital and ask the clerk to issue a covering letter and a document asking the OASI in DPO Office to check your physical standard, take the advertisement with you showing the minimum required standards.
Step 2:
Now take that letter and form to DPO office in OASI branch, SP or DPO will call a committee of atleast 3 personals to check your height and Chest. They will write it down on that form marking with PASS or FAIL.
Step 3:
Now again back to DHQ for your vision test ( not mandatory but necessary for overall clearance) in Ophthalmology Department. Doctor will check your vision and you have to get the same certificate signed by Head of Department.
Step 4: Now take the signed form to MS of DHQ or THQ whatsoever and get it signed , Your Document is ready to dispatch now.
You have to send Original Physical Standard Document with your application
Any other piece of paper OR any signed and stamped document showing physical standards is not acceptable

problem of kashmir.. Its not like that.. simply go to any govt hospital. Ask them to make a nominal medical mentioning height, weight and chest. This is it.
