Yes. Only 1 set of attested photocopies with name stamp of BS-17 or above officer is required.
Private experience should be mentioned on form and also attach experience certificate of private job. It is best to print Annex A on letter head of private organization. If not possible then send available experience certificates.

What to study for interview? What don't you ask such questions here? Ap loag bus minute se cheezu k lye pareshan rahte hen like kitni pics chahye, front pe attest hoga ya back pe, send kaha aur kis tarha karna hy, attested kaha se karvani hy, experience dyna ha ya nae etc etc. Mind na karna please you all are respectable.
