If your name is in this list, it doesn't mean that you're pre-selected. This list is only for some candidates who didn't submit their relevant docs properly at the time of application. So, keep your fingers crossed! The actual list of pre-selected candidates is coming!

Yes, DPC should be on FPSC 's prescribed format available on the site. After downloading it's hard copy you should get it filled and singed from the authority. Submit this signed Form along with other documents to FPSC. Save it's photocopy in your record.
