How many photos we have to attach while sending the AD forms. There are two empty boxes for the picture. Do we have to send an extra picture other than those 2 pictures.

Is there anything mentioned on the application form that we need to send the domicile copy along with application? I didn't come across any such instructions. It only requires application form, with photographs, NIC copy and original deposit slip. Thats what my understanding is.

Yes. You're right.
I advice, you should apply.
It is mentioned only result awaited candidates cannot apply.
Anyhow, as far as I know you should apply, but try to manage for domicile ASAP because it takes too long to be made.
However I think you can submit your parents' domicile as well if deemed necessary (but not sure about it).

I sent the application along with 2 photos, copy of cnic and orig nts copy of bank slip BUT forgot to write branch code and date in the column on first page though date and code are mentioned on the bank slip. Is that ok? Please advise.
