In my view you should get your old medical certificate attested from MOFA when attesting other documents. In this way you don't need to go to MOFA twice and you will have both MOFA attested old certificate and embassy designated hospital's new certificate.

I think going by road will take up 5 days of tiring journey and about 35k to 40k. Srilankan Airlines takes around 42k for Karachi to Beijing. PIA is taking about 62k (plus 5% service charges if you buy ticket using cash from PIA facilitation center) for Lahore/Islamabad to Beijing.

Interestingly somewhere I read that PIA offers 5% discount to students if they have proof. If they are traveling to join their respective university then admission offer letter is also acceptable.
