Already mentioned in detail. Only medical board and they issue certificate on the same day when board is scheduled. Even, it is possible different camp hospitals can follow different protocols.

Fresh eye sight checkup from any eye specialist is also required by candidates besides blood tests etc.

Undertaking of surety bond can be printed in Rs. 50/- stamps.
Bro don't worry about this. Weight will have no effect. These tests are usually required to prevent patients of hepatitis B/C, T.B e.t.c because these diseases spread easily and other people get the infection interacting with them.
Chest xray, a urine test and 2-3 blood tests are all that are required.

At this stage only acceptance letter is required to be submitted. Better attach a copy of your offer letter as well.
