I already told you what I knew, if you are not satisfied then please use your friend "google" and see the embassy notification yourself.
Short answer: Only embassy designated hospitals are acceptable for visa process if you want to apply at embassy in Islamabad. Medical certificates from designated hospitals need not to be attested from MOFA.
