I am applying to several chinese universities and all of them require a physical examination form before or after the submition of the application but most universities will accept the form even after the application submission. However there is one university i.e university of electronic science and technology of china that requires the physical examination form at the time of submission of the application.

My question is that from where can i get the form filled in Lahore. I heard there was a chinese hospital in Lahore from which if we get that form filled we would not have to get it attested later on. There is also another post on this forum about the same topic but the link in that post pertaining to the name of the hospital sadly does not work. I would really appreciate if any one would tell me the name and the address of this chinese hospital through which i can get my physical examination form filled so i wont have to get it attested later on.
