Default Some more queries on form

What does this clause of the form means?

19. Full particulars of the near relations, if any i.e. father, mother, brothers, sisters, first uncles and first relations in law who are/were in Government service of Pakistan or elsewhere, specifying the posts held and where posted.

Moreover, do we need to mention our professional qualifications as well?

If one is working in a private organisation, i think there is not need to provide any docuemnt in this connection. Is it?

For example as my Father Is X.E.N Gulberg Lahore . So i'll mention this in the form that he is my father , his name , designation X.E.N and place of posting i.e gulberg lahore.
