On right hand side you will see a column Contact us
There you will get address and phone number

Dear Initials means short signature (to save space) or even you can do your full signature on each change. I hope you have not made so many mistakes.

No issue, you will get many chances later to update your bio data, once you pass the exams. So my request is to concentrate on exams instead of such minor issues.

Even i have also made two changes in the application form but they were minor. I expect FPSC wouldn't have any issue with such changes (Annual income of father and date for declaration of bachelor result)
