From policy perceptive the number one problem is population, there are too many mouths to feed and our economy can not take this weight. Our population is growing at a rate so high that we are unable to respond adequately. For instance, despite HIV/AIDS outbreak in rural Sindh there is no family planning programme being implemented. This is leading to more HIV/AIDS, especially in the newborn.
The inequalities are are becoming a problem because of the growing population. People who are stuck in poverty and they have no means of producing any income are having more children. These people also face a nutritional deficiency, but they are still having children causing high stunting rates.
More people means more waste, which is directly linked to environment.

The only problem that is devastating Pakistan in every respect is "Economy". The best measures may promptly be taken in this regard, so that all other issues be settled down accordingly.

Leadership can change anything..
