Dear fellows! If anyone amongst you could enlighten us about the 'domicile certificate accepted on FPSC's prescribed form'? As I've found myself in a quandary over this problem as that kind of form is not available on the commission's website and there are Seven candidates out of a total of Thirty Eight (amounting to 18%:1/5th of the the total lot) who are faced with this thing. Any clue from anywhere?

Brother, FPSC never ever demands a domicile certificate on its prescribed form as there has never been nor will be any such form. Domicile certificate is used, as you know well, to certify your Province and Distt of residence and it is issued by relevant Distt authorities. No one has got to get it issued again and again except for when it goes missing or such thing. Now to what you need to do: Brother you need to get a fresh DPC (Departmental Permission Certificate) on FPSC's prescribed format from your current department and submit it to FPSC. Hope that helps.

Thank you very much fellows for your worthy comments. This matter pertains to my comprehension of that letter, while I was taking it to be two items, in fact, there's just one. The error is regretted. All that the commission is asking for is a DPC with the candidate's district of domicile mentioned on it. I fear that I am getting old
