I completed my said degree in 2012. how to file an appeal ? is there any form to fill ?

Deposit FPSC Chalan of Rs. 100 as fee for representation. Write to the Secretary FPSC, Subject: Representation against rejection U/S 7(3)(a) of FPSC ordinance 1977 for case F4-19/2015-R.
Narrate the facts, basis of rejection and then your arguments/basis of representation. Attach the original TR and all the necessary documents along with representation and send it, before expiry of 30 days from the date of letter notified. you will also receive the rejection letter by post. In case of satisfaction by the commission, your candidature will be restored.

Anyone received letter from FPSC in respect of pre-selection?

as they said that candidates are informed through registered post..

Dear All, My name was in Documents Required List but now my name is disappeared form both Pr-selected and Rejected List. please guide me what i can do in this regard at least to know what happens to my Application.

Because you are not rejected on pretext of any thing but you are below the merit threshold. Thats why you are neither in pre-selected nor in rejected.

Your name may be in low merit/failed 35394 candidates , which is not shared on site
