Well there is no such form available in hospitals. I had to print it on a plain paper and then the DMS signed it. Anyhow, that's just a formality simple statement will do.

"List below (Pages-02 to 08) are advised to furnish the attested copies (with name stamp of attestation officer) of following requisite documents within 15 days of placement of this Notice on FPSC website through Registered Post/Courier, to determine their eligibility."

It applies to all the documents, even if it doesn't, no one is going to be rejected on this basis. In pre-selection list, you're asked to bring any missing document along with yourself at the time of interview.

Plus my height has not changed over a month or two and I believe my chest measurement is still the same. I won't torture myself to go through the same process for such a certificate that is not even a certificate in real sense. I have sent a copy for PO and I'm going to send a copy this time as well.

Can I go to any DHQ for this certificate?
Because I am from Mianwali but living in Lahore. Please someone tell that which hospital in Lahore is authorised to issue medical certificate??

In simple words, kisi bhe hospital chaly jao bhae jan. ap to kuch zada he tension ly rhy. You can get it from any govt hospital whether its peshawar or karachi, no matter.

1. Which are the authorized medical authorities?

2. Do we have to send the original medical proof or attested copy of the medical document?

3. Being a Computer Science graduate, how do I present the proof of MS Office literacy? I have not done any particular courses in MS Office applications but I have been using those applications for a long long time?

I asked them on e-mail and they said we can get it from any government hospital. Now, i don't know the pattern or format of the required document.
