Yes, that is SVA form. After that, there is supposed to be security clearance and a visit from some officials at any of your given address.

If salty khan, khan Mohammad and Mohsin Hasan have received sva forms for 2 posts posts for fata and GB, it seems the ministry has sent forms to most likely and reserved candidates.
